New Clients

To begin the process, please complete our Contact Form. Once we receive your information, we’ll create a client chart and share the intake paperwork electronically, making it easy for you to complete from home. Please keep an eye on your email for this paperwork. If we are billing insurance we will verify benefits and, depending on your insurance requirements, a PCP referral may be required to have on file. If a referral is needed, we’ll guide you through the process. Please note that your child’s intake paperwork and any necessary insurance verification must be completed before we can schedule an appointment!

WE ARE NOT CURRENTLY ON A WAITLIST!!! We expect to get new clients scheduled with a week or in some cases within the same week.

*If you are the parent of a previous Wobbly Beginnings client and the new client form does not submit for you, please reach out through the client portal messenger or email jessica@wobblybeginnings.com to have a new chart set up an additional child.
